Indiana HEA 1385 (2024) prohibits balance-billing for out-of-network ambulance services; insurer reimbursement is the lesser of the locally-set rate or 400% of Medicare. Excludes Medicare, Medicaid, the state employee plan, and ERISA self-funded plans.
