Verify before acting

Statutes and regulations change. State balance-billing laws are revised annually. The decoder uses our best understanding of the current rule set as of the "last verified" date on each citation, but for any decision that affects your bill or your appeal rights, verify the rule directly with the primary source (45 CFR Part 149 on eCFR; state DOI on the state's primary URL) or consult a healthcare-billing attorney or patient-advocate.

No fabrication

Every load-bearing claim on this site cites a primary source. Where a state's ground-ambulance posture has not been primary-source verified, the state row carries a [VERIFY] flag and the state DOI consumer-protection page is the recommended starting point. We do not invent procedural deadlines or state-law rules from memory.

No fake reviewers

The reviewer-attribution block on any page renders only when a real credentialed reviewer (a healthcare-billing attorney or a certified patient-advocate) has signed off on the content. We do not fabricate reviewer names or credentials.
